Requirements for International Transportation of Human Remains (Ashes)

The following documents shall be required for the issuance of a transfer permit for ashes:

(a)    An official ID picture (Social Security or Passport).

(b)    An official certificate of cause of death (Original or authenticated copy of the Death Certificate). 

(c)    An official certificate of cremation (Original or authenticated) by the person/ funeral parlor authorized to cremate the remains indicating the manner and method in which the body was cremated.

(d)    Flight detail for the transportation of the human ashes. 

Departure from Origin:
Airline, Flight number, Departure location, Time of departure,

Arrival in:
Airline, Flight number, Arrival location, Time of arrival,

(e)    Next of kin/ Funeral Home that will be in care of the ash.

(f)    Final place of internment of ashes or disposition.

(g)    All above documents or information must be submitted to the Director of Health Services either in person or through the following mediums:

NB.
A Copy of Death Certificates, Cremation Certificate, and Transit permit shall accompany the shipment of human ashes.  The container should be air tight and package properly for safe shipment.

Final provisions

All requirements as set forth herein by the Ministry of Health must be met before the necessary approval can be granted.
